Prevention Navigator:

Bebashi – Transition to Hope seeks a Prevention Navigator to work with clients in underserved communities; primarily African-American and Latino communities.

The Prevention Navigator will determine eligibility to participate in Navigation services.  In addition, the selected candidate will screen and test clients for HIV, STIs, and pregnancy, conduct short-term counseling sessions, conduct sexual health educational presentations and workshops, and provide referral services to clients for psychosocial services.

MAJOR DUTIES AND RESPONSIBILITIES

  • Determine client eligibility to participate in Navigation services.
  • Screen and test clients for HIV, STIs, and pregnancy.
  • Conduct short-term counseling sessions with clients.
  • Facilitate educational presentations and workshops on sexual health.
  • Provide referral services for psychosocial support and resources to clients.

DESIRED QUALIFICATIONS AND SKILLS

The qualified candidate will possess:

  • A bachelor’s degree in public health, human services or a related field;
  • 2 years of experience working with high-risk HIV clients and HIV/AIDS education,
  • Certification in HIV/AIDS prevention and testing;
  • Willingness to travel to underserved communities in Philadelphia;
  • Superior verbal and written communication skills;
  • The ability to work with people from all walks of life and ethnicities;
  • Must have a working knowledge of MS Office products;
  • Bi-lingual in Spanish a plus
